There are several things patients will need to bring during their first visit to our office.
 
Before departing for the appointment, take a moment to create a list of all medications you are currently taking, both prescription and over-the-counter. Doses and frequency should be included.

Please also bring your insurance card, referral (if required), and any x-rays or other medical records specifically requested.  
 
Upon arrival at the office, check-in with our front-desk personnel. You will be asked to fill out a patient information and medication/allergy sheet.
